Important Announcement Regarding Preparticipation Physical Exams

The new Scholastic Student-Athlete Safety Act (SS_ASA)(N.J.S.A. 18A:40-41.7) requires that:

1. The Student-Athlete Cardiac Assessment professional development module (PD module) be completed by the physician, advanced practice nurse (APN), or physician assistant (PA) who performs a student-athlete's annual physical examination prior to the student's participation on a school-sponsored interscholastic or intramural athletic team or squad in grades six through 12.

2. Upon performing a physical examination, the physician, APN, or PA shall sign the certification statement on the Preparticipation Physical Evaluation (PPE) form attesting to the completion of the PD module. this certification can be found on page four of the PPE form.

Attention 2024 Spring Season Athletes (Baseball, Softball, Boys Tennis, Golf, & Spring Track)

If you would like to participate in a Spring Season sport, please submit the following paperwork to the school nurse.

1. Sport Physical Packet - If the student-athlete does not have a valid sports physical on file with the HS nurse (last physical exam on file is prior to March 11, 2023), he/she must submit the Pre-participation Physical Evaluation Forms in the Sport Physical Packet. All incoming 9th grade athletes who are playing a HS sport for the first time must submit a new Sport Physical Packet. *Please read the information regarding the Scholastic Student-Athlete Safety Act on this page before seeing your physician for a pre-participation physical evaluation.

2. Health History Update Questionnaire - If the student-athlete has a valid sports physical on file with the HS nurse (a physical exam after March 11, 2023), he/she must complete the Health History Update Questionnaire. This form can be found at the bottom of this page. Please return a signed copy to the Nurse's office.

3. Asthma/Food Allergy Forms - ONLY student-athletes with asthma and/or food allergies must submit these forms. All pages must be filled out properly and signed by the parent/guardian and doctor where indicated, and notarized where needed. These forms only need to be submitted once every school year. *These forms can be found at: https://flhs.flboe.com/offices/nurse.

*All appropriate forms must be submitted to the school nurse in order to participate in a Spring Season sport.

SportsWareOnLine

Attention All Parents/Guardians: Fort Lee High School now uses an injury tracking software program called SportsWareOnLine. Included in this program are 8 consent forms that need to be electronically signed before your child can participate in his/her sport. Please login to SportsWareOnLine (www.swol123.net) and electronically sign the 8 consent forms. If you do not have an account yet, you may download the "SWOL Introduction Letter to Parents" document located at the bottom of this page and register an account for your child.

Injury Protocol

Athletes must report any injury to their coach, athletic trainer, or school nurse. If an athlete sees a doctor for an injury, the athlete must provide written documentation clearing the athlete to return to school and sport activity. This documentation must be handed in to the athletic trainer or school nurse upon the student's return to school. The Student Medical Report can be downloaded below or picked up in the athletic training room or the nurse's office.
